About The Role:

This role involves the provision of high quality unit based clinical pharmacy services including the opportunity to undertake credentialing for Partnered Pharmacist Medication Charting (PPMC) and other advanced scope of practice programs, and to train in specialty clinical streams such as critical care and medical specialties. Possible rotations to operational services including dispensing and compounding services and the Ballarat Regional Integrated Cancer Centre (BRICC) may occur.

Key responsibilities include active contribution to

• Provision of comprehensive clinical pharmacy services (assessment, intervention, advocacy and consultation)
• Effective patient flow principles
• Mentoring and clinical supervision of learners at all levels including pharmacy students, interns, residents and other Grampians Health staff
• Pharmacy and multidisciplinary team education programs and meetings

To Be Successful In This Role:

• Bachelor of Pharmacy Degree
• Current registration with AHPRA
• Commitment to improving patient outcomes through medication management
• Membership to an appropriate professional body – Society of Hospital Pharmacists of Australia or Pharmaceutical Society of Australia
• Prior clinical pharmacy experience in a hospital is preferred
